AmPac Business Questionnaire
This form is designed to help the AmPac team to learn more about your business and your business journey. Provide as much detail as possible. If this information is available on your website, please indicate.
AmPac Affiliate Questionnaire
This form is designed for individuals who own or control affiliate businesses. Affiliate businesses are not the businesses seeking a loan, but rather a business that the principal borrower owns more than 50% interests or otherwise controls the business’ day to day operations.

Form 413 - Personal Financial Statement
This form is for documenting your personal financial statement.
Form 912 - Personal History Statement
A misdemeanor or even a felony, unless the case was against a child, is still eligible for SBA financing. All information must be Disclosed, with the application. Should information be uncovered that was not disclosed, it could result in disqualification from SBA loan programs.
Form 1624 - Personal Finance for MicroLoan and CDFI
This form is for documenting personal financial information for Micro Loan or CDFI loans.
Form 1919 - Disclosure for Micro Loan and CDFI
A misdemeanor or even a felony, unless the case was against a child, is still eligible for SBA financing. All information must be Disclosed, with the application. Should information be uncovered that was not disclosed, it could result in disqualification from SBA loan programs.
What to Expect
AmPac's loan process involves four steps
Day 1-5
Step 1
AmPac and our bank partner (if real estate or equipment loan) meet with you to understand your business and financing objectives and begin collecting financial information.
Day 6-10
Step 2
You, the business owner, completes a checklist of financial information to include personal and business tax returns and/or business plan and projections.
Day 10-35
Step 3
The bank and AmPac concurrently underwrite the loan based on financial information submitted. We ask questions of you, the borrower and collect additional financial information as needed and obtain approval from loan committee and/or SBA. We order an appraisal or environmental reports as appropriate.
Day 35-45
Step 4
The bank and AmPac will ensure that all company operating documents are collected and then close and fund the loan.
